Use the drop down list to select the facility you are requesting. These relate to our calendars. Choose the specific facility, not site locations such as elementary, middle school.
The date you wish to reserve the facility. Check the calendar below to ensure the facility is available on the days you are requesting.
The date range you want the facility available to you including set up and tear down time. AM and PM is required for times.
Choose the specific facility from the drop down list. This may seem redundant, but is required. This list does not correspond to calendars. DO NOT use the New Facility button, choose a facility from the list. Requests will be denied if a facility from the list is not selected. If you want to request a facility that is not listed, please contact Debbie McManus for assistance.
Describe any special arrangement of tables and chairs that is needed. More than one facility may be requested if needed for the same dates and times.
List full name and organization that is represented (FCCLA, Basketball, HS Music Dept etc...).
Provide a description of the activities that will be conducted during the event including any fees for event participation. For ex: in addition to typing a description you may paste contents of a flyer, website links, attach a copy of a flyer or pictures, or upload a sketch of facility layout.
